In some cases, children as young as 7 may benefit from interceptive orthodontic treatment — carefully timed treatment that guides proper growth and prevents bigger problems later.
It is important to emphasise that we do not recommend or provide orthodontic treatment for children under the age of 7. At such a young age, there is no strong clinical evidence or research to justify orthodontic intervention, despite what you may read online from those advocating treatment as early as 3 or 4 years old. In our opinion, this is overtreatment and not in the best interest of the child.
For ready children, early treatment is usually limited and focused. The most common approach is palatal expansion (using a small device called an RPE), sometimes combined with short sections of braces or aligners to move a few teeth. This type of treatment is designed to create space, balance the bite, and support proper development.
Some children who struggle with breathing issues — such as snoring or paediatric sleep apnoea — may also benefit from palatal expansion. In certain cases, widening the palate can help improve tongue posture and support better breathing. It’s important to note, however, that orthodontics alone does not cure breathing problems. Often, the main causes are enlarged tonsils or adenoids, which may need to be addressed by an ENT specialist. Dr Erfan Salloum works closely with two leading ENT consultants in Dublin, ensuring children receive the most comprehensive care possible.
In selected cases, myofunctional therapy (specialised exercises to retrain the tongue and oral muscles) can be an important addition. Dr Salloum collaborates with an excellent myofunctional therapist to support children who need this extra step.
We are also mindful of the ongoing discussion — and sometimes misinformation — around “airway orthodontics.” At Lusk Orthodontics, our philosophy is based on evidence-based care. As Dr Salloum often explains, the truth lies in the middle: not every child needs an expander, and not every expander will solve breathing issues. But for the right patient, early intervention can make a significant difference.
Other reasons for early orthodontic treatment include crossbites (when the upper and lower teeth don’t meet correctly). These can occur at the front or back of the mouth and are usually managed with expansion, sectional braces, or aligners. In some situations, additional appliances such as mini-screws, Motion appliances, or elastic bands may be used. While face masks were once commonly used, advances in modern orthodontics mean these are rarely needed today.
Finally, in rare cases, a tongue tie (restricted tongue movement) may affect normal tongue posture and growth. Where appropriate, Dr Salloum will refer children to trusted colleagues for a simple release procedure, with or without orthodontic support, depending on the child’s needs.
